The following documentation will be required along with your application:
- Your current Curriculum Vitae
- A copy of your current AHPRA registration
- Contact details for two referees
- Copy of the certificate of currency of your Professional Indemnity insurance showing:
- Amount of cover
- Category of insurance
- Copy of Radiation Licence (if required)
- Period of insurance
- Working with Children check
- Covid-19 vaccination status
Subject to receipt of all required information, all applications are reviewed for approval by the SPH Medical Advisory Committee (MAC).
